Who can use OhioHealth MyChart?

Any active patient of our practice is eligible to activate and use OhioHealth MyChart.


Is there a fee to use OhioHealth MyChart?

No. This service is free to our patients.


How do I receive an activation code for OhioHealth MyChart?

If you have a scheduled appointment or visited this physician office, you may already have an OhioHealth MyChart account. To access OhioHealth MyChart for the first time, you need an activation code number. You have 2 options to receive an activation code:

  • Call our office or ask the medical receptionist when you arrive for your activation code.
  • Or, go to OhioHealth MyChart to request your activation code.
